I/We, parent(s) or guardian(s), by submitting this form, agree that I/we have read and understood the following:

I/We understand the potential risk in sport activities and give my/our consent and agree to release, indemnify and hold harmless TORONTO HAWKS SOCCER CLUB (THSC) --the Club, the team, officials, volunteers, league or governing soccer association, the City of Toronto and Toronto District School Board, its employees, agents, officers, directors and elected officials, from any and all claims, damages, cost and expenses that may be incurred, arising from any injury or damages sustained by my child or by me and arising from my child’s participation in all Toronto Hawks Soccer Club programs.

I/We hereby grant the Toronto Hawks Soccer Club the right to photograph and videotape my child while participating in the soccer program for reproduction and use on the THSC website and future communications and marketing materials developed and distributed on the Club’s behalf. I grant the THSC and its agents the perpetual and non-exclusive right to use my child’s image and first name in communication, alone or with other images and materials, and waive any right to inspect or approve the finished communication. I understand and agree that I will receive no monetary compensation for such use.

I/We understand the Club retains the right of player/team placement. Players may be traded within the Club and no special request is guaranteed.

Refund Policy: Refund requests must be submitted in writing. Full refund will only be granted for medical reasons (with doctor’s note). Other refunds will be subject to a $45 administration charge. Refund requests must be submitted before September 25th for the indoor program and May 15th for the outdoor season of the same year as registered. After the above dates, no refund will be granted.

Notice: For indoor season, all players must wear shin pads and indoor soccer shoes, and for outdoor season, all must wear shinpads and soccer shoes, in all games and practices.
